Dear Parents,

We want to inform you that the Health Department, Govt. of Sindh, will be visiting Hira Foundation School to raise awareness about Polio Vaccine Drops.

Recommended age: Under 5 years

Validity of Consent: August 2026 – May 2027

Please indicate your preference below regarding whether you want or don’t want your child to receive the Polio Vaccine drops.

[ ] I want my child, _______________, of grade ______, to be administered the Polio Vaccine drops.

[ ] I don’t want my child, _______________, of grade ______, to be administered the Polio Vaccine drops.

Kindly complete the consent form and return it to us by Monday, 31st August 2026.
